Certified Community Behavioral Health Clinic

Bridges is launching a new initiative called a CCBHC or Certified Community Behavioral Health Clinic, which will offer enhanced access to behavioral health and other integrated services in Milford, West Haven, Orange and Stratford.

New services through the CCBHC initiative will include:

• Clinical services at a new location in West Haven, CT (expected to open this Fall)

• Assertive Community Treatment for individuals in need of a higher level of support including case management, peer recovery coaching, medication management and outpatient services

• Expansion of mobile crisis services for adults and children that include overnight hours and on site evaluations

• Mobile primary care services in Milford and West Haven, CT including health screenings; hepatitis and HIV testing.

These service are available to all Bridges' clients. Please contact your clinician, prescriber or case manager if you are interested in learning more.

This expansion of services is made possible through federal funding from the Substance Abuse and Mental Health Services Administration. Bridges is committed to providing comprehensive person-centered services that are accessible and convenient.
